Comments: The fellowship is a mixture of both clinical experience and clinically oriented research. We are a busy regional practice in a tertiary medical center at the largest hospital in the Kansas City metropolitan area. Fellows receive a broad exposure and will become proficient to numerous types of blocks. We utilize both nerve-stimulator and ultrasound-guided approaches to peripheral nerve blocks. We have a strong emphasis on continuous catheter infusions and a sizable home infusion patient population following ambulatory surgery. The fellows are instrumental in the education and overseeing of anesthesiology residents during their regional rotation. There is no call obligation associated with the fellowship. |

The Columbia Regional Anesthesia Fellowship offers a one-year program with a diverse clinical experience. Fellowship training is designed to advance the candidate beyond technical proficiency with ultrasound-guided peripheral nerve blocks (with/without a nerve stimulator) and continuous catheters, to become experts in perioperative pain management as well as in the management of complex patient populations. Fellows receive exposure to a variety of techniques for orthopedic, vascular, breast and abdominal surgery. Currently 2000 peripheral nerve blocks and 400 peripheral nerve catheters are performed annually with anticipated continued growth. Approximately 60-80% of the time will be spent in the clinical arena with the remainder being protected research and administrative time. Fellows are expected to participate in research as well as actively contribute in a weekly regional anesthesia journal club. The majority of clinical time is spent in a dedicated 4-bed monitored holding area performing and teaching regional anesthesia techniques to the anesthesia residents. Additional teaching responsibility includes instruction to medical students in the anatomy lab. The opportunity also exists to round with the Acute Pain Service.
Fellows have no attending or call responsibilities. There are voluntary moonlighting opportunities if desired.

Comments: Our program is designed to do two things: First, produce graduates that are experts in all of the peripheral nerve blocks for upper and lower limb, as well as other blocks such as thoracic paravertebral, ilioinguinal, TAP block, etc. Fellows receive training in both nerve-stimulator and ultrasound-guided techniques. Secondly, we strongly emphasize clinical research and as such fellows spend at least 2 days a week as non-clinical, designing and conducting clinical studies that are continually being generated. Fellows have the opportunity to conduct workshops at our annual NYSORA meeting in December, which is attended by anesthesiologists from around the world, and at our other NYSORA workshops and symposiums held both here in New York and abroad. In addition, other opportunities such as the presentation of abstracts and workshops at other meetings (e.g. the ASRA annual spring meeting, the ASA annual meeting) are typically part of the experience. Teaching of residents is also in important part of the program for our fellows. Ultimately, our goal is to have our fellows go on to academic programs and further the field of regional anesthesia by passing their knowledge and skills to the next generation.
*Collaborative Decision Date Agreement
This Program adheres to the Regional Anesthesiology and Acute Pain Medicine Fellowship Directors Group's "Collaborative Decision Date Agreement". This agreement mandates that no candidate will be required to accept or decline an offer from any participating Fellowship Program prior to September 1st of the previous academic year.
The participating Programs hope that this initiative will improve the application process for candidates.

Comments: The teaching, research and clinical applications of regional anesthesia are essential components of the Department of Anesthesiology and Perioperative Medicine at OHSU. We offer advanced training in all techniques of orthopedic and Regional Anesthesia (RA). Our patients benefit from a wide range of RA techniques from traditional to cutting edge for surgical anesthesia and/or acute pain control.
Fellows will be active participants of the RA team. Currently all RA techniques of the upper and lower extremities, as well as truncal blocks, are routinely performed at OHSU. Single-shot blocks and catheters are placed daily for adult and pediatric patients and many of our patients (over 1,000 in 2008) are discharged home with perineural infusions (including interscalene, supra/infraclavicular, lumbar plexus, femoral, saphenous and sciatic).
Our RA faculty is large and diverse. Several of our staff are fellowship trained in RA or pain management. We have active basic science and clinical research programs and are involved in teaching RA at national and international meetings.
While the majority of our blocks are placed using ultrasound guidance, fellows will also become proficient using traditional techniques.
Fellowship duties are not limited to the placement of neuraxial or peripheral nerve blocks, but also include patient selection and education as well as coordination of the RA team's daily activities with nursing staff, anesthesia technicians, the pharmacy, surgeons, colleagues and the acute pain service (APS).
Fellows will participate in at least one of the RA team's ongoing research projects, including manuscript preparation and data presentation at department and national meetings.
Fellows will present formal lectures to anesthesiology residents and medical students.
At the completion of the training, fellows will be able to perform a comprehensive array of RA techniques, set up an ambulatory or hospital-based APS and conduct research to advance the specialty of RA.
This is currently not an ACGME approved fellowship and there is no sub-specialty certification.

Fellows rotate between the University/Huntsman Hospitals and the Orthopedic Hospital. One day per week will be spent on academic/research pursuits. The other two days per week are spent working as a faculty in the operating rooms.
Fellowship Highlights:
Fellows will assist in placement of peripheral nerve blocks and catheters, as well as neuraxial anesthesia with emphasis on thoracic epidurals. Blocks will be placed under the guidance of an experienced acute pain faculty member. As experience increases, fellows will have the opportunity to perform a number of procedures independently.
Fellows will learn to set up and manage an acute pain service, including management of complex pain patients, mutli-modal analgesia, coordination of care with chronic pain/palliative care, management of APRN's and expertise in billing and financial management.
In our practice, peripheral nerve blocks and catheters are placed with only ultrasound guidance. We do not provide any training with nerve stimulation techniques. This fellowship represents a unique opportunity to become an expert in ultrasound skills, ultrasound anatomy an ultrasound needle guidance.
In addition to substantial clincial work, fellows will participate in bi-annual fresh cadaver dissections.
The University of Utah has a volume of approximately 1000 Single shot blocks, 1400 perineural catheters and 700 neuraxial bloacks per year.
Fellows will actively participate in the education of residents: Residents rotate for 1 month at a time at each location. Fellows will work closely with the residents in both procedures and education.
Fellows will serve as the sole attending supervising the acute pain service (for a week at a time) several times toward the end of their fellowship year.
Fellows will be expected to participate in a research project during the fellowship year with the goal of publication of their data in one of the premier anesthesia journals.

The Department of Anesthesia and Pain Management of the University Health Network and the University of Toronto offers a 12-month fellowship training program in Regional Anesthesia and Acute Pain Management at the Toronto Western Hospital. Our training program aims to confer both the clinical proficiency and academic knowledge that a consultant regional anesthesiologist in modern anesthetic practice requires.
A total of 4 fellowship places are offered each year, beginning in July. Applications should be made at least 18 months in advance, and shortlisted candidates should be prepared to travel to Toronto for a site visit and interview. Final selection decisions are generally made 1 year in advance of the start date.
Clinical Experience
The Toronto Western Hospital performs approximately 3,000 regional anesthesia procedures each year. These are organized around a block room model.
Fellows will gain both organizational skills related to regional anesthesia as well as technical expertise in a variety of peripheral nerve block techniques performed for orthopedics, plastics, and general surgery. Under the direct supervision of an attending regional anesthesiologist, fellows will learn to perform interscalene, supraclavicular, infraclavicular, and axillary brachial plexus blocks, as well as distal forearm blocks of individual nerves. Lower extremity blocks routinely performed at our institution include femoral, sciatic, popliteal, and ankle blocks. Fellows will also gain expertise in continuous catheter perineural infusion techniques for both the upper and lower extremities, as well as neuraxial blocks. Fellows will master the fundamental skills of ultrasound –guided intervention, which will allow them to apply it to virtually any procedure.
Fellows will also participate in the Acute Pain Service rotation and non-regional anesthesia cases during the training year. The Acute Pain Service manages approximately 2,000 patients per year including some referrals with challenging chronic and cancer pain.
On non-regional days and on call, fellows will have clinical exposure to the other surgical specialties at Toronto Western Hospital, which include neurosurgery, major spine surgery, general surgery, bariatric surgery, urology, and interventional radiology.
Research Experience
Regional anesthesia fellows will be allocated to spend 20% of their time in research and educational activities. There is a strong academic focus at Toronto Western Hospital, and fellows are expected to demonstrate commitment to academic work. Under direct staff supervision, regional anesthesia fellows are expected to initiate at least one research project and prepare at least one research protocol during the training year. There is ample opportunity to participate in research projects already underway in various stages from data collection to manuscript preparation, as well as to assist in the authorship of monographs and book chapters. Fellows are expected to present their research at national and international meetings as well as publish their work in high-impact journals. Our current areas of research include ultrasound-guided peripheral nerve blockade and neuraxial blockade, education and training in ultrasound-guided regional anesthesia, complications of regional anesthesia, outpatient pain management techniques, and anesthetic applications of diagnostic ultrasound.
Education
Regular Regional rounds are held at the departmental level throughout the year which will include journal discussions, case discussions, and didactic presentations. Fellows are expected to participate in these rounds to gain experience in presentation and teaching. The group conducts several regional anesthesia workshops throughout the year in which fellows are expected to participate, first as attendees, and then as faculty.
Teaching Experience
Fellows are expected to assume a teaching role and instruct both junior and senior residents in regional anesthesia techniques. Fellows will also have opportunities to further develop teaching skills as faculty at many of the educational initiatives offered by our Department of Anesthesia, including our quarterly Hands-On Ultrasound for Regional Anesthesia Workshops, the annual Regional Anesthesia and Pain Medicine Conference, and bi-annual International Symposium on Ultrasound in Regional Anesthesia in Pain Medicine.
