Physicians applying for a fellowship program in regional anesthesiology/acute pain medicine must be currently enrolled in, or have completed, an accredited anesthesiology residency program. Each individual program may have additional requirements such as medical licensing. The information published here has been supplied by the individual institutions. EMAS offers fellows the opportunity to gain the experience, technical skills, and business acumen to become a successful and highly sought after private practice Regional Anesthesiologist. This fellowship is centered on fast-paced ‘real world’ practical training with a focus primarily on ambulatory orthopedic surgery centers. There is a strong emphasis on learning safe, effective RA techniques for a full range of surgeries, but also on how to become a business-minded surgery center medical director. This includes learning about evaluating and optimizing workflow, resource management, hiring practices, developing & presenting business plans, billing and reimbursement, managing payroll, submitting an RFP, and contract negotiation.

Fellows will work at the renowned Campbell Clinic Orthopedic Surgery Centers and at our other fast-paced and challenging outpatient and inpatient clinical environments. EMAS performs over 150 ambulatory total joints per month, and fellows will expect to perform between 6,000 – 7,000 procedures during the 12 month program. No nights, weekends, holidays. Great work/life balance. Will not compete for procedures with residents. In order to maintain and further develop core anesthesiology practice skills, fellows will function 80% as fellows and 20% as faculty. Opportunities to expand interests into inpatient regional anesthesia applications including continuous peripheral nerve catheters. Participation in academic research is available but not a requirement.

The University of Tennessee Graduate School of Medicine Pain Medicine Fellowship is an ACGME-accredited 12-month (starting and ending in July) training program designed to qualify physicians for specialty certification in pain medicine by the American Board of Anesthesiology.

The regional anesthesia and acute pain medicine fellowship at the University of Tennessee Medical Center in Knoxville, TN is a non-accredited fellowship providing a unique experience for fellows to learn and work alongside a private practice group within an academic medical center. Our program provides a robust clinical exposure to a multitude of regional anesthesia procedures in both the inpatient setting as well as a brand new, fast-paced, ambulatory orthopedic surgery center. Fellows will also participate in our Multimodal Acute Pain Service alongside our subspeciality pain anesthesiologists and mid-level providers that manage complex acute pain inpatients with a variety of techniques including lidocaine infusions and ketamine infusions. In addition to providing a comprehensive education in regional anesthesia and acute pain medicine, fellows will also serve as the staff anesthesiologist supervising operating room cases and fulfill an Instructor faculty position with the Graduate School of Medicine.

Vanderbilt's Regional Anesthesiology and Acute Pain Medicine fellowship was established in 2009 and accredited during the first cycle of accreditation for the specialty in the Fall of 2016. The program accepts 2 fellows each year, with expansion to 3 fellows per year under consideration by the ACGME (determination in Jan 2022). The fellowship is split between several sites. 

Primary Site:

Vanderbilt University Medical Center (VUMC): a large, quaternary care hospital and level 1 trauma center and a national Top-5 trauma center by volume.

Secondary Sites:

Vanderbilt Health Belle Meade and Vanderbilt Surgery Center, two fast-paced ambulatory orthopedic surgery centers

Monroe Carell Jr. Children’s Hospital, a VUMC adjacent children's hospital with acute and chronic pediatric pain programs

Vanderbilt Psychiatric Hospital

Kijabe Hospital, Kijabe, Kenya, the site of an optional international regional anesthesia elective

Fellowship Rotations include:

Regional Anesthesia and Acute Pain

  • > 7,000 blocks per year across all sites
  • High volume of epidural and peripheral nerve catheters, including management chest and extremity trauma
  • Home peripheral nerve catheter program
  • Focus on perioperative medicine, including enhanced recovery (ERAS) programs
  • Training in multimodal analgesia and opioid minimization
  • A multidisciplinary team, including faculty, fellows, residents, & NPs  

Ambulatory Regional Anesthesia:

  • Ambulatory surgery centers with a private practice feel
  • Focus on speed, efficiency, and single-injection and peripheral nerve catheters
  • High-volume ambulatory total joint replacement center
  • 8-12 weeks spread through fellowship

International Regional Anesthesia (optional, 1 fellow per year)

  • 4-week, regional anesthesia-focused rotation at a hospital in Kenya
  • Vanderbilt has a full-time faculty member there throughout the year as medical director of their operating suites
  • Heavy use of neuraxial and peripheral regional anesthesia
  • ACGME-approved rotation, minimizing logistical barriers
  • Opportunities for travel and expense funding

Pediatric Pain

  • Large, academic children’s hospital
  • Hybrid acute/chronic/regional pain experience
  • A focus on pediatric ERAS

Addiction Psychiatry

  • Education in buprenorphine initiation and management
  • Learn to manage opioid use disorder and withdrawal
  • First-hand experience of the personal toll of the opioid crisis
  • Gain perspective on what is at stake when patients are placed on perioperative opioids

Adult Chronic Pain

  • Inpatient chronic pain service and outpatient cancer pain clinics
  • Participate with basic procedures as well as neurolytics and implants

Oromaxillofacial Surgery

  • Perform intraoral nerve blocks for dental procedures
  • A distinctive 2-week experience offered by few other fellowships

Obstetric Anesthesia (optional)

  • Over 5,000 deliveries per year with a focus on high-risk obstetrics

OR Anesthesia

  • 2-3 weeks paired with attendings supervising 3-4 rooms of OR anesthesia
  • Maintain OR skills and practice airway, vascular access, and supervising skills
  • Perform blocks for patients in those ORs

Aside from the clinical experience, the curriculum is focused on a broad range of areas, leveraging our superb faculty to provide exposure to an array of topics including deep dives into anatomy, practice, and service management, enhanced recovery principles and practice, classic and emerging techniques for regional, informatics, education techniques, etc.

In summary, Vanderbilt combines a world-class clinical experience with training in the breadth of regional techniques, a deep focus on education, and clinical rotations that cannot be matched.

 

Approximate Salary: $72,887 with $2,400 CME funds; opportunities for internal moonlighting after 4 months if meeting fellowship milestones (TN medical license required). Additional funding for international rotation and conferences.

    Description

    The department of anesthesiology at Vanderbilt University is proud to direct the institution’s first and only multidisciplinary pain management fellowship. Accredited by the ACGME in 2008, our fellowship combines extensive experience with advanced interventional techniques in addition to rotations in regional anesthesia, addiction psychiatry, neurology, radiology, neuroradiology, neurosurgery, physical medicine and rehabilitation, cancer pain, and palliative care. Trainees in the fellowship program will be extensively exposed to strategies of pain management by both anesthesiologists and faculty from other disciplines. The fellows, regardless of their primary specialty, should be competent in pain assessment, formulation, and coordination of a multiple modality treatment plan, integration of pain treatment with primary disease management and palliative care, and interaction with members of a disciplinary team.
